Since the last edition was published, the pace of progress in basic science and clinical medicine has become even faster, and completely new modalities have been added in the biotechnologic, genomic, and informational sciences. The fourth edition of the Textbook of Respiratory Medicine is admirably positioned to report and analyze these developments, but because of the rapidity of change, is being published after a 5-year cycle instead of the usual 6 years. As with its predecessors, the Textbook has been reorganized, with the addition of several new chapters plus the consolidation or removal of old ones to make room for recently acquired information and to avoid repetition. Seventy-eight new authors, with an increasingly international presence, have joined other distinguished colleagues to write about subjects ranging from positron emission tomography scanning to severe acute respiratory syndrome to video-assisted thoracoscopic surgery. To provide the necessary expertise for these chapters, we have often paired complementary coauthors: a pulmonologist with a specialist from another field, or with a thoracic surgeon, or with a basic scientist. We are deeply grateful to these dedicated authors for their outstanding contributions.
